The Case Management /Service Coordinator role is an integral part of our service delivery. As part of a wider Service management team, the Service Coordinator manages own caseload whilst working within a team to support collective service management and scheduling needs. It demands an energetic, experienced and highly organized individual, with experience and knowledge of the Social Services/Education/Health Sectors, experience in service coordination and account management is essential.
