Person Directed Planning (PDP)

wm+a is committed to the social inclusion of persons with developmental disabilities/dual diagnosis. We work from a person-centred perspective that promotes individualized approaches, and supports individuals to be independent and have choices in their lives.  The individuals we support have the right to receive services that are based on respect and dignity, accessibility, safety and security, accountability and sustainability. Person Directed Planning is one way that we demonstrate this at wm+a.

 

wm+a offers trained facilitators with the objective of assisting individuals with developmental disabilities over the age of 18 to identify their life goals and find community connections and services that are meaningful to them.  Together, a plan is developed to bring those life goals to reality.

Passport Funding can be used towards PDP as per MCCSS guidelines. In line with the MCCSS guidelines for the delivery of PDP services, wm+a has taken all the necessary steps to ensure quality of service delivery and a meaningful outcome to our clients. PDP facilitators have received specialized training in PDP and they work with each person from a strength based, person centred approach.

PDP facilitators meet with individuals independently or with their families/support team in order to create and actualize a personalized plan for that individual. The facilitator will listen to each person as the unique individual they are, learn how they want to live, and discover what supports are needed to move toward a life they consider meaningful and productive.

The planning process, done virtually or in person, is empowering as it puts the individual at the centre of this ‘Life Plan’:

 

  • builds on a person’s specific abilities, skills and interests
  • takes into account health/wellness, community, work/education/volunteer, leisure, daily living and more
  • supports individuals to be involved in their community
  • identifies and strengthens the person’s support network
  • identifies all informal and formal supports and services needed
  • creates a living document that can be reviewed and changed needed
  • Supports the individual to direct the plan and to have a voice in their life

 

An important part of the process is to ensure that the PDP facilitator is well matched to the individual. The uniqueness of the goals are as important as ensuring that the right facilitator is supporting that journey.
